Cardiovascular manifestations of systemic lupus erythematosus: the significance of heart failure

The involvement of the cardiovascular system is a typical manifestation systemic lupus erythematosus (SLE), which determines high level mortality and disability patients. A serious clinical problem development heart failure (HF), frequency in SLE 3–4 times more than population. this pathology complex process that occurs under influence autoimmune inflammation associated with damage (pericarditis, myocarditis, endocarditis, сcoronary artery disease, myocardial infarction), disorders cardiac conduction (various arrhythmias), atherosclerosis, arterial hypertension, pulmonary thrombosis against connected bleeding (especially antiphospholipid syndrome), traditional risk factors, as well negative effect anti-rheumatic therapy. Mostly HF subclinical form preserved ejection fraction, detected using instrumental methods 60% management patients requires early diagnosis pathology, to do this, various diagnostic are used (particularly, echocardiography speckle tracking imaging technique) identification biomarkers such NT-proBNP. therapy based on maximal reduction o f activity disease due rational pathogenetic therapy, also control factors – antihypertensive use statins prevention venous thrombosis.
